If you have never seen me before, you will need a New Patient Appointment.

Please note that New Patient Appointments require a $50 non-refundable deposit to be finalized.

Once your appointment request is approved you will receive an email with an invitation to the Patient portal to complete needed intake forms, as well as an email with an invoice to make your deposit and secure your appointment.
